- Description
- Offers residential and lifestyle supports including a wide range of personalized supports to people living in the community - primarily outside of their family home.
Many continue to receive support within group home settings, others reside in home-share situations or apartments.
People who access services receive supports only as needed to encourage their independence.
Supports may range from direct personal care * health care * housing help and skill building to a consultative model of support for those people involved in the Supportive Living Program. - Eligibility
