- Description
A regional resource that works with individuals who frequent the Emergency Department for mental health or addiction issues or experience opioid dependency issues on a short term basis. Ensures linkage and connection to primary health care, and ongoing mental health and social services.
The USAT team provides support through various points of service (e.g. NHS, WMS, Segue Clinics, CMHA Safe Beds and New Port Treatment Centre) as well as home visits.
A collaborative initiative of Quest Community Health Centre, Niagara Health System (NHS), Canadian Mental Health Association (CMHA) of Niagara and Community Addictions Services of Niagara (CASON).- Eligibility
- Individuals who:
- Experience opioid dependency issues (current or within the last 12 months); and/or
- Frequent emergency room visits for mental health or addiction issues
