- Description
- Provides support to individuals with serious, ongoing mental health needs.
Clinical Case Management services are for individuals who are:- Living with ongoing and serious mental illness
- Having difficulty managing medication and their side effects
- Needing help with life skills and activities of daily living
- Requiring education and counseling for themselves and their family
- 60 and older
- Offers education, support and counselling regarding mental illness, medication management and connection to community resources
- Need help accessing appropriate services
- Are not in a long-term care facility and does not have dementia
