Niagara Region Mental Health - Case Management


Description
Provides support to individuals with serious, ongoing mental health needs.

Clinical Case Management services are for individuals who are:
  • Living with ongoing and serious mental illness
  • Having difficulty managing medication and their side effects
  • Needing help with life skills and activities of daily living
  • Requiring education and counseling for themselves and their family
Geriatric Mental Health Case Management services are for individuals who are:
  • 60 and older
  • Offers education, support and counselling regarding mental illness, medication management and connection to community resources
  • Need help accessing appropriate services
  • Are not in a long-term care facility and does not have dementia
Eligibility
Clinical Case Management
  • Individuals experiencing ongoing and serious mental illness
Geriatric Mental Health Case Management
  • Adults 60 years and older
  • Not in long-term care facilities and does not have dementia
