Community Paramedicine
- Description
- Community Paramedicine programs support vulnerable populations within the community outside of regular 9-1-1 operations, services include:
CP@Clinic- seniors wellness clinic (location varies based on need)
- social and healthcare clinic provided to persons experiencing homelessness
- in-home supports and primary care for seniors with chronic conditions and who cannot not attend other forms of primary care
- supporting individuals within the community who rely on emergency services and healthcare to support health and social needs
- specialized services, community partnerships and referral pathways to enable assessment, assistance and advocacy for patients and caregivers within the community
