Supportive Housing Program


Description
  • Attendant Care Services provide non-medical assistance with activities of daily living in the home, such as dressing, bathing, eating, and helping with range of motion
  • Supportive Housing Program provides Attendant Care Services to individuals who require availability of/access to 24/7 staffing (Housing Application through United Counties Leeds and Grenville Social Housing required)
Eligibility
Individuals must: • be 16 years of age or older • have provincial health coverage and a valid health card • have a permanent physical disability or functional limitation which results in the inability to independently complete personal care (e.g. toileting/bowel routines, washing/bathing, dressing, eating, medication) • require assistance with personal care from MODC as a result of this physical disability/functional limitation • be able to self-direct care (i.e. the individual is capable of managing, determining, and communicating when and how assistance is needed) • not be in receipt of, or expecting receipt of, insurance funds for provision of attendant services. For Supportive Housing, individuals must: • be unable to have their needs met effectively in the community on a scheduled/unscheduled basis (i.e., through MODC outreach or other existing community resources) and require availability of/access to 24/7 staffing • have support needs, including existing medical condition(s) that can be adequately met by MODC onsite staff within available hours or by local community health services • be able to be safely left unattended within a community-based shared service model Visit the website for all eligibility requirements.
