- Description
- Delivers one-on-one care to people in their own homes, schools or workplaces.
Helps with daily activities such as:- taking medication
- personal grooming and hygiene
- rising and retiring
- dressing and undressing
- bathing and washing
- bowel/bladder services
- range of motion exercises
- essential communication tasks
- Eligibility
- Applicants must :
- be 16 years of age or older
- have provincial health coverage and a valid health card
- have a permanent physical disability or functional limitation which results in the inability to independently complete any of the following examples of personal care routines: e.g., toileting/bowel routines, hygiene, washing/bathing, dressing, transfers/positioning, eating, medication, tracheostomy care, and require assistance from MODC due to this physical disability/functional limitation
- be able to self-direct their own care (i.e. the individual is capable of managing, determining, and communicating when/how assistance is needed
- have a service location that will have the physical features necessary for service delivery without significant risk of serious physical harm to MODC staff, or to help identify reasonable steps to reduce the risk of harm
- not be in receipt of, or expecting receipt of, insurance funds for provision of attendant services
