Day Program

Supports Italian, Spanish and Portuguese speaking older persons living with Dementia (PLWD) and/or mild forms of Alzheimer's Disease

  • provides meaningful, culturally and linguistically relevant therapeutic activities including drawing, rosary, singing, baking, cooking and exercise to assist PLWDs maintain cognitive, social, emotional and physical health
  • nutritious meals
  • falls prevention exercise
  • educational workshops
  • caregiver education and support
  • personal care support
  • medication administration
Eligibility
Italian, Spanish or Portuguese speaking older adults 55 years and older diagnosed with early stages of Dementia or Alzheimer's Disease
  • functionally impaired, but not homebound, requiring supervision and monitoring, and preventive/diagnostic/therapeutic/rehabilitative/palliative care or services
  • capable of participating in the program and its activities; requiring minimal assistance to participate
  • capable of managing self care
