- Description
Interprofessional team provides comprehensive services for care partners and their family members living with dementia
- services offered in-person, virtually and by telephone
- group and clinical services help care partners manage the day-to-day care of a person living with dementia
CARERS Program - groups help care partners cope with and reduce emotional stress and improves their ability to deal with demands of caregiving
- focuses on practical skills and emotional supports needed to care for people living with dementia
- teaches problem solving techniques, a structured approach to address problems
- coaches care partners in communication skills through the use of simulation
- provides strategies for keeping a meaningful relationship with the person living with dementia
- 8 weekly sessions, 2 hours per session, maximum 6 care partners in group
Reitman Centre at Mount Sinai Hospital is the provincial hub for the Enhancing Care for Ontario Care Partners Program, which is the exclusive provider of CARERS groups in over 50 locations across Ontario.
- Eligibility
- Care partners and family members caring for someone living with dementia; clinical services and groups for care partners are available to spouses, partners, adult children, siblings, and other family or friends
- Application Process
- Referral from a health-care provider is encouraged, but care partners may also contact the program for services directly. Program also accepts referrals from other clinical services and community agencies
