- Eligibility
- Individuals 18 years and older with a progressive life-limiting illness able to maintain independence with personal care and the administration and management of medication, including pain pump and oxygen
Clients must also be ambulatory, including with the use of mobility devices, and cognitively able to engage programs and activities
