- Eligibility
- Persons 18 years and older with a primary diagnosis of aphasia resulting from a stroke, brain injury or illness, or with a diagnosis of Primary Progressive Aphasia (PPA)
- Application Process
- Referral from a speech language pathologist or medical professional
- Documents Required
- Completed referral form (found on website); see separate referral form for PPA (on website); Speech-Language Pathologist (SLP) report required for individuals diagnosed with aphasia due to stroke, brain injury or illness; medical report required for those diagnosed with Primary Progressive Aphasia
