Description
Outpatient exercise and education programs for those who have suffered stroke, cardio-vascular disease or health issues that put them at high risk for heart disease such as diabetes or high blood pressure
  • integrated approach including physical assessment, exercise training, patient education and risk factor interventions
  • HEALTh Program for women being treated for or recovering from breast cancer including exercise, education and peer support
  • Diabetes, Exercise & Healthy Lifestyle Program
  • aftercare such as Heart Health for Life
Eligibility
Persons with a history of heart disease or stroke and those who are at high risk for heart disease, including those with diabetes
HEALTh Program - persons being treated for or recovering from breast cancer
