  • comprehensive geriatric assessment including memory
  • education on chronic disease self-management
  • teaching about dementia and behavioural care
  • advance care planning
  • referral to community supports
  • access to personal support and home safety assessments
  • de-prescribing and medication adjustment
  • initiation of treatment for dementia
