416 Community Support for Women


Description

Health and wellness program, one-on-one support, and a drop-in program for women (cis and transgender) who are experiencing or are at-risk of experiencing homelessness, and/or are facing mental health and substance use challenges

Health and Wellness Program

  • medical clinic on-site, with referral

    • family health practitioner

    • ICHAs Regional Mobile Nursing Program

    • psychiatric clinic

    • chiropodist

Case Management Program

  • individualized assessment and goal setting

  • supportive counselling

  • education and support around daily living skills

  • crisis prevention and intervention

  • network building and the coordination of care teams

  • substance use support

  • mental health advocacy and support

  • advocacy and referral to other services

  • services provided at the client's preferred location

Drop-In Program

  • nutritious meals

  • Community Support Workers can assist with referrals and connections to shelters and addictions and mental health crisis support

  • scheduled group programming

    • opportunities for learning new skills and building a sense of community through social interactions and discussion groups

Eligibility
Female identified individuals 16 years and older who are experiencing or are at-risk of experiencing homelessness, and/or are facing mental health and substance use challenges
Coverage area: Downtown Toronto
