Description

Supports at-risk seniors transitioning from hospital to other care settings in collaboration with existing outreach services

  • develop behavioural support plan to manage behaviours in hospital and contribute to successful transitions between care settings
  • build knowledge and capacity within hospital teams related to behaviours
  • assistance with applications to long-term care or community settings
  • service process includes intake assessment, comprehensive behavioural assessment, development and implementation of support plan
  • each patient is supported by one case manager and one personal support worker
Eligibility
Individuals 55 years and older who are: experiencing behaviours related to cognitive impairment, mental health and/or addiction; no longer require acute care and are not in delirium; are designated Alternate Level of Care (ALC) or are at risk of becoming ALC; consent to a referral being made
