Community Program
6-12 week program held in clients home or community location once or twice a week- assessment required
- individualized needs and goals plan
- goal areas include money management, education or employment, community orientation and travel training, daily living skills and stress management
Supportive Housing
Cooperative environment at Edwards Manor, located at Royal York and Bloor- 24 hour support services
- capacity 10
- Eligibility
- Supportive Housing: adults 59 years and older who have sustained mild to severe brain injury who require long term supported living environment; must provide medical documentation confirming diagnosis
- able and willing to live in community setting
- able to work on short and long term goals and commit to the program
- do not present a harm to themselves or others
- are able to interact safely while out in the community
- must be independent for all transportation and personal support requirements
