Description

Children's In-home Services program provides medical and/or rehabilitation services to children and youth

  • nursing
  • personal support services
  • occupational therapy
  • physiotherapy
  • speech language pathology
  • dietetic services
  • social work

School Health Support Services program provides services in school to assist students, up to age 21, with medical and/or rehabilitation needs that are beyond the range and responsibility of school staff

  • nursing services
  • information and referral services
  • coordination of services
  • dietetic services

For students in private school or students who are home schooled services may also include:

  • occupational therapy
  • physiotherapy
  • speech language pathology
  • personal support services
Eligibility
Children and youth birth-30 years
Students must have a valid Ontario Health Insurance Plan card
Needs of the student cannot be met on an out-patient basis
Students require at least one of the professional services available through Ontario Health atHome
