- Home At Last
- help aging adults and adults with special needs to settle in safely at home following a stay at hospital
- certified and specially trained Personal Support Workers drive or accompany the patient home
- provide personal care, prepare a small meal, pick up groceries/prescriptions and perform light housekeeping as required the day of discharge
- follow up with phone calls and/or visits to check on client's well-being and make referrals to other community support services
- provides Personal Support Worker (PSW) accompaniment to high-risk older adults who require support to attend primary care appointments or assessments
- Eligibility
- Aging adults and adults with special needs or those who are isolated

- Referrals generally come from hospital staff but clients can self-refer by phoning
