- Ongoing, comprehensive and individualized mental health services in the client's home or in the community including treatment, rehabilitation and support
- Eligibility
- Adults with schizophrenia, psychotic disorders, and bipolar disorders who have had a minimum of 60 hospital days in the previous 2 years
Residency Requirements: South Mississauga; South of Burnhamthorpe Rd until West side of Mavis Rd; from the East side of Mavis Rd, South of Eglinton Ave - Application Process
