HIV Client Support Services Program
- Description
Access to support, prevention, and community based interventions; capacity building and training for community partners and service providers a part of all agency programs
HIV Client Support Service Program- case management services
- one-on-one counselling; needs assessment
- HIV pre and post-test counselling
- referrals to healthcare professionals, social workers and legal services
- support with access and adherence to treatment and medications
- transportation support to medical appointments; accompaniment
- connections and coordination of support among peers
- help with ODSP, Trillium Health Program, OW and CPPD application
- wellness checks - calls to clients to follow up and check ins
- system navigation and advocacy; settlement services
HIV Community Engagement
- We Belong - bi-weekly drop-ins
- HIV Treatment update workshops - monthly Lunch and Learn
- health promotion and capacity building training for HIV clients
- capacity building and training for community partners and services providers
- Eligibility
- People living with or affected by HIV/AIDS; Equity owed communities: African Caribbean and Black people, Indigenous people, newcomers, refugees, immigrants, migrant workers, international students, women, youth, people who use drugs, 2SLGBTQ+ (2-Spirit, Lesbian, Gay, Bisexual, Trans, Queer and Questioning)
