Description

Transitional Care Centre -- support with activities of daily living in a safe environment

  • post-operative and recovery (max. 14 days)
  • respite and convalescent care (max. 30 days)
  • short term transitional care model (STTCM) (max. 50 days)
  • Assisted Living -- personal support workers (PSWs) provide individualized attention and intensive support with all living activities including bathing, dressing, medication reminders, case management and referrals, transportation

Respite Companionship -- workers provide in-home non-medical care and companionship for seniors and adults with disabilities

Eligibility
Adults 55 years or older who can walk safely and independently 20-30 meters with or without gait aid (ie. cane or walker), site is not wheelchair accessible
Application Process
Only by referrals from home and community services, Central West
Self referrals and/or their caregiver, post-operative/respite/convalescent care
Documents Required
Referral application for waitlist and admission determined by CCS
Agreement to be signed and dated prior to admission

Transitional Care Center -- $45 per night includes semi-private room, personal care, telephone, television with basic cable, and meals
Respite Companionship -- $20/hr for 3-hour minimum, with a $50 assessment
