Community-based mental health and supportive housing services enabling individuals with identified psychiatric illness to live independently in the community

Assertive Community Treatment Teams (ACTT)
Multidisciplinary teams assist adults 16-65 years who are experiencing severe, persistent mental illness; includes community integration, psycho-education, advocacy to access health care service and vocational opportunities, and support with issues impacting wellness, such as homelessness, substance use, and/or involvement with judicial system
  • assistance in client home or community setting

Early Intervention
​Mental health counselling and community-based intensive support for individuals 14-35 years experiencing symptoms of psychosis; education on mental health and substance use issues, links to services and resources

High Support Program
For individuals with complex mental health and co-occurring issues

Housing Services
In-Sted (In Short Term Emergency Diversion) short-term, community-based mental health and addiction services to reduce need for visits to emergency departments, includes linkages to other community-based services

Integrated Seniors Team Program
Supportive and health services enable high-risk seniors to live independently in their homes

Peer Support Initiatives
Includes social enterprises staffed and managed by individuals living with a mental illness, including Social Coffee Bean, DC Cleaning

Problematic Substance Use
Support for individuals with complex addiction issues, harm reduction approach
Eligibility
Individuals 16 years and older with mental illness, varies by program
