Assertive Community Treatment Teams (ACTT)
Multidisciplinary teams assist adults 16-65 years who are experiencing severe, persistent mental illness; includes community integration, psycho-education, advocacy to access health care service and vocational opportunities, and support with issues impacting wellness, such as homelessness, substance use, and/or involvement with judicial system- assistance in client home or community setting
Early Intervention
​Mental health counselling and community-based intensive support for individuals 14-35 years experiencing symptoms of psychosis; education on mental health and substance use issues, links to services and resources
High Support Program
For individuals with complex mental health and co-occurring issues
Housing Services
In-Sted (In Short Term Emergency Diversion) short-term, community-based mental health and addiction services to reduce need for visits to emergency departments, includes linkages to other community-based services
Integrated Seniors Team Program
Supportive and health services enable high-risk seniors to live independently in their homes
Peer Support Initiatives
Includes social enterprises staffed and managed by individuals living with a mental illness, including Social Coffee Bean, DC Cleaning
Problematic Substance Use
Support for individuals with complex addiction issues, harm reduction approach - Eligibility
