- Description
A Housing-Focused program offering mobile supports throughout South Simcoe to individuals, couples and families experiencing or on the verge of homelessness, providing access to life saving resources, supplies, referrals, and supports through brief case management. Access to basic needs, personal hygiene, and harm reduction supplies, as well as access to food and food resources.
The Outreach team is mobile, seven days a week, throughout the five municipalities that make up the area known as South Simcoe (Innisfil, Essa, Adjala-Tosorontio, Bradford/West Gwillimbury and New Tecumseth). The team makes regular stops at established locations in each municipality several times weekly.
The team will also make individuals appointments to meet up with participants at a location convenient for all.

- No restrictions, anyone impacted by homelessness or low income.
