  • early diagnosis, treatment and support
  • healthcare team works closely with seniors and their families to manage medical conditions including memory problems
Eligibility
  • Age Requirement 65 years and up
  • All patients must be registered with the Barrie Family Health Team Doctors/Nurse Practitioners
  • individuals living with memory loss
  • individuals age 70 years and over with complicated medical concerns with or without cognitive issues
Application Process
Referral from a Family Health Team Physician or Nurse Practitioner
