- Eligibility
- Age Requirement 65 years and up
- All patients must be registered with the Barrie Family Health Team Doctors/Nurse Practitioners
- individuals living with memory loss
- individuals age 70 years and over with complicated medical concerns with or without cognitive issues
- Application Process
- Referral from a Family Health Team Physician or Nurse Practitioner
