Patient-centered education and on-going management for individuals with Type 1 and Type 2 diabetes
- patients receive initial assessment and are provided with follow-up appointments as indicated by their diabetes status
- education focuses on supporting healthy lifestyle changes for improved diabetes self-management, understanding principles of healthy eating and physical activity, importance of blood glucose monitoring, understanding targets for control, role of oral medications and insulin
- registered ADP insulin pump program site for Barrie and surrounding communities
- Eligibility
- Patients with Type 1 or Type 2 Diabetes who are registered patients of the Barrie Family Health Team
