Community Paramedic Service


Description

Non-emergency, community-based service supports vulnerable patients living independently in the community. Aims to reduce 911 calls, emergency department visits and hospital admissions.

Services include:

  • in-home assessments
  • direct referrals to community partners
  • chronic disease exacerbation management
  • remote patient monitoring
  • direct communication with primary care providers
  • point-of-care diagnostic tests
Eligibility
Dufferin residents who require additional assistance while living indecently , including high-risk clients living with complex medical issues such as diabetes, COPD and CHF.
Application Process
Email or fax form https://www.dufferincounty.ca/county-services/paramedic-services/community-paramedic-program/ Referrals from patients, family members or service providers will be reviewed upon receipt to determine eligibility for program. Community Paramedics will make initial contact with the patient within 24 hours after receiving a referral to discuss.
