- Description
Helps hospital patients successfully make the transition from hospital to home. Provides a Personal Support Worker to help clients "settle in" the day they are discharged from hospital. Services may include:
- accompanying/driving a client home from hospital
- picking up medications, medical equipment and/or groceries
- making a small meal and doing light housekeeping
- conducting a safety assessment
- providing personal care
Operates as a partnership between Peterborough Regional Health Centre, Ontario Health atHome (formally Home and Community Care Support Services Central East), and Community Care Peterborough
- Eligibility
- Adults who are frail or have special needs and may benefit from additional support settling in at home upon discharge from hospital
18 year(s) and up;
