This service is a partnership between Northumberland Hills Hospital, Campbellford Memorial Hospital,Bridge Hospice,Trent Hills Family Health Team, Northumberland Family Health Team, Port Hope Northumberland Community Health Centre, Ontario Health atHome - Central East, Palliative Pain and Symptom Management Consultation Service
