An interdisciplinary community outreach team providing clinical and non-clinical supportive care. These include:

  • Palliative support
  • Caregiver support
  • Vigil support
  • Community education
  • Disease management
  • Health system navigation
  • Inter professional consultation
  • Palliative education, information, and training
  • Grief and bereavement counselling

This service is a partnership between Northumberland Hills Hospital, Campbellford Memorial Hospital,Bridge Hospice,Trent Hills Family Health Team, Northumberland Family Health Team, Port Hope Northumberland Community Health Centre, Ontario Health atHome - Central East, Palliative Pain and Symptom Management Consultation Service
