- Description
The Outreach Primary Care team is an interprofessional collaborative group that includes a physician, nurse practitioners, nurses, outreach workers, and a social worker, that delivers full scope primary health care and social care to vulnerable clients who are experiencing homelessness and insecure housing.

- When you arrive at the Health Centre please expect: 1. To be Screened at the Front Door Your temperature will be taken Screening questions will be asked 2. You will be expected to wash your hands. 3. You will be expected to physically distance by staying 2 metres or 6 feet away from the person next to you. 4. If you have a mask, please wear this to your appointment. If you don't have a mask, you will be offered a fabric mask and will need to wear this while in the building
