Description

Coordinated Service Planning (CSP) provides support to families with children/youth with multiple and/or complex needs

Coordinated Service Planning will:

  • be a warm line of support and a clear point of contact for families
  • support and promote the family's voice through the service planning process and collaboration with other service providers and community partners
  • help prioritize and monitor the progress of goals through a coordinated service plan
  • assist with service navigation and referrals to other supports as prioritized by the family's needs
  • assist with transitional planning needs
Eligibility
Children and youth with complex and/or multiple special needs, birth to exit from school (up to age 21) who have at least two service providers involved in the their care (including schools, therapists, community supports and medical professionals)
