- Children and youth under 18 years of age who are a resident of Haldimand or Norfolk counties and diagnosed with a developmental disability

- Referrals can be made by a family member, another agency or an individual with consent of the family by calling 519-587-2441 or 1-800-265-8087 and asking for Contact at ext 350
