A community-based pulmonary rehab program to assist individuals to better manage Chronic Obstructive Pulmonary Disease (COPD)
Program consists of:- weekly education and exercise sessions (10 weeks)
- follow-up by a multidisciplinary team (12 months)
Team includes:
- Coordinator/Social Worker
- Kinesiologist
- Occupational Therapist
- Dietitian
- Smoking cessation support
- Eligibility
- Individuals with Chronic Obstructive Pulmonary Disease (COPD)
- Application Process
- Referral from a healthcare provider or contact to initiate a self-referral
