- Clinic for individuals with issues or concerns with memory
- service consists of 2-hour assessment by a Registered Nurse, Alzheimer's Society of Brant staff and a Physican
- driving concerns will be addressed at this assessment
- Eligibility
- Residents of Brant County or the City of Brantford ages 60 years and older who have a primary care provider
- Application Process
- All referrals to the Memory Clinic, as well as the Geriatric Clinic (Seniors Assessment Service) must be directed to Central Intake via fax to 1-855-406-2163 utilizing the referral form accessible at www.rgpc.ca/centralintake/
