Youth Community Mental Health Program


Description

Serves street-involved youth ages 16-21 in Hamilton, meeting youth at various residential and non-residential programs within Good Shepherd Youth Services and the Street Youth Planning Collaborative (SYPC).

Program offers:

  • Comprehensive Mental Health Assessments
  • Evidence-Informed Individual/Family Counselling and Therapy to support Youth and their Families (*families as defined by the youth) with their mental health and wellness
  • Connections with consulting Psychiatrists, Psychologists, & Primary Healthcare Practitioners
  • Care Coordination, Case Management, and Advocacy
  • Support for 2SLGBTQ+ Youth
  • Peer Supports
  • Peer Family Supports
  • Wellness Toolkit Programming (*DBT-informed)
  • Staff Capacity Building and Training for Good Shepherd Youth Services staff and SYPC Agencies.
Eligibility
Open to street-involved youth, ages 16-21 years who are living in Hamilton or the surrounding area.
