Community Care Durham

The Assisted Transportation Service is currently closed to new requests as it has reached full program capacity. All existing appointments will be honoured as scheduled. At this time, agency anticipates beginning to schedule new requests in the Spring of 2026

Provides clients with rides to and from essential destinations

• medical appointments
• shopping, errands, and appointments
• to access Community Care Durham (CCD) programs
• drivers can be CCD staff or volunteer drivers

Access to Primary Care

• provides a Personal Support Worker for clients who need to attend medical appointments and have mobility or other related challenges

Accessible Vans

• wheelchair accessible vans driven by CCD staff for clients with mobility issues
• staff push client up ramp, secure wheelchair and bring down ramp
• client has to be able to wheel themselves to ramp or have a caregiver do so
• must travel with escort if cannot travel alone
