ID Clinic


Description
The ID Clinic provides assistance to obtain ID including: health card, Canadian birth certificates and in some cases Verification of Status. The ID Clinic also provides information and referral about replacing other ID. For individuals experiencing homelessness KDCHC can register individuals for OHIP without address information.
Eligibility
The ID Clinic focuses on supporting people experiencing homelessness. The program also supports individuals with a permanent address in Kitchener or Waterloo who are living on a low, fixed income.
Application Process
By appointment: call 519-745-4404 ext 207, or drop-in to 44 Francis Street South if there is no access to a phone.
Documents Required
Clients are encouraged to bring any supporting documents they have, but in most cases the ID Clinic can assist acquiring ID with no supporting documents.

Hours of Operation
The ID Clinic meets clients by appointment throughout the week. Call to make an appointment, or drop in if there is no access to a phone. The ID Clinic also offers outreach services at several locations in the community. Outreach locations include: HOF, St. John's Kitchen, King Street Shelter, encampments, Better Tent City, Women's Crisis Services, Motels, Ray of Hope, Grand River Hospital and St. Mary's General Hospital.
