This program helps youth, who have a history of involvement with Children's Aid Society (CAS), find and access adult services and resources and make the transition
Youth In Transition Worker helps youth to make plans and decisions regarding:- housing
- education
- health care
- life skills
- employment
- personal goals and well being
- Eligibility
- Youth aged 16 years-24 years who have had a history with the Children's Aid Society (CAS)/ Family & Children's Services (FACS)
