- Description
- Helps make the transition from hospital to home
- Provides transportation and the services of a Home at Last (HAL) Attendant (PSW)
- Once at home the HAL Attendant can provide assistance with groceries, medication and medical supplies pick up, light housekeeping, laundry and meal preparation
- HAL Attendant will provide the client with a package of community support services
- Followup within 24-48 hours by HAL supervisor
- Eligibility
- Lives in area serviced
- can direct own care
- ambulatory, transfer with one person assist
- Age Requirements: 55 year(s) and up
