VON Assisted Living for High Risk Seniors aims to help people maintain their independence and remain in their own homes for as long as possible without being institutionalized in a hospital or long-term care home.

- Application Process
- Clients require a referral from a health care or social service provider. Clients interested in receiving Assisted Living Services can contact the Central East LHIN directly at 1-800-263-3877 to be assessed to see if they meet the criteria for eligibility.
