Description

On-call mobile crisis service for seniors; multidisciplinary team provides

  • crisis de-escalation
  • short term intensive case management
  • supportive counseling
  • harm reduction
  • mental health and addiction support
  • geriatric assessments
  • connections to primary care through dedicated nurse practitioners
  • behaviour supports
  • in-person assessment and support within 24-48 hours of phone referral; service offered in clients' home or in their local community


Partnership between WoodGreen Community Services, LOFT Community Services, Reconnect Community Health Services, Haven Toronto/St. Michaels Homes, South Riverdale Community Health Centre

Eligibility
Older adults 65 years and older who are in crisis (experiencing a distressing change) and not coping well with independent living in the community; seniors with cognitive, physical or mental health challenges in need of crisis support; also older adults 55-64 years with geriatric concerns in need of crisis support

Consent from client is not needed for the initial visit, but is required for ongoing support
Application Process
Call for referral; Toronto Seniors Helpline staff will assess for eligibility and urgency; no waitlists
