Description

Provides services and resources to families with children who are at risk of, or have, a delay in their development; infant and child development consultants work with families to provide early identification and intervention to support their child's development

Screening and Assessment

  • monitor a child's overall development
  • provide verbal feedback and written letters or reports to parents and service providers with parental consent
  • plan for transition to school or other services

Service Coordination (Case Management)

  • help families access needed supports and services
  • clarify roles and responsibilities when families are involved with multiple service providers
  • collaborate with community partners

Preterm Services

  • based on identified child risk factors for children born under 35 weeks gestation
  • consultations every 1-3 months or as necessary
  • regular developmental screenings
  • helpline available for information and support, 905-568-3806 ext. 3201

Information and Support - development-focused parent education sessions and community education workshops

Discharge Planning - to continue to meet the needs of the child, may facilitate referrals to other community programs to support ongoing developmental needs

Eligibility
Children birth-6 years of age diagnosed with a developmental delay or are at risk of having a developmental delay. Risk factors include being born prematurely, having complications at birth, prenatal substance exposure, diagnosed medical condition, or having a sibling with Autism Spectrum Disorder
