YMCA Hamilton Youth in Transition Worker Program
- Description
Program providing support for youth who have been involved with Family and Children Services. Helps youth to transition to a more independent lifestyle by developing and implementing a personal plan, and providing help navigating adult service systems and connecting with community supports and resources.
- Eligibility
- Open to youth ages 16 to 24 who are or have been involved with Family and Children Services.
- Application Process
- Phone, text, or email the Youth In Transition Worker for information. Referrals are accepted by a community partner or self-referral.
