The Dental Clinic is available for individuals or families with low income or who do not have dental insurance.

Application Process

Apply Online to HSO

Mail with printed application form to: Ministry of Health Ontario Seniors Dental Care Program Station P, PO Box 159 Toronto, ON M5S 2S7

Assistance with completing the application is available by calling the Haliburton, Kawartha, Pine Ridge District Health Unit at 1-866-888-4577.
