Support related to Diabetes including
- individual assessment and nutrition counselling
- instructions on blood glucose meters and diabetic medication/insulin management
- group education sessions on topics such as blood glucose management and lifestyle changes
- Eligibility
- Persons with Type 2 Diabetes or Prediabetes, and pregnant persons with diabetes, including Gestational Diabetes

- Documents Required
- Bring Ontario Health Card or other health insurance coverage information, blood glucose meter and log book and list of medications and supplements to each appointment
