This program is intended to meet the needs of low-income individuals to meet an emergency or unexpected health need

Examples of items that can be covered:

  • emergency dental
  • emergency prescription drugs
  • dentures
  • vision care
Eligibility
Must be a resident of Brantford or Brant County
Application Process
Call to find out about qualifying at 519-759-3330 ext 6594 (press option 2 for local office)
Documents Required
Documents required for all family members over the age of 18 living with the applicant:
  • proof of address
  • proof of identification
  • proof of household income (all family members aged 18 and over)
  • Notice of Assessment (if not available or income has greatly changed since the prior year, other current documentation can be used, pay stubs, cheque stubs, bank statements, etc)
  • professional referral/prescription
  • quotes (if applicable)
  • direct deposit Information (if applicable)
