- Description
Housing Program
- works with chronically homeless tenants to access and maintain alternative supportive housing in shared accommodation
- safe space for individuals experiencing deep poverty with a history of chronic homelessness and often the long-term effects of trauma and substance use
- serves tenants through a harm reduction, eviction prevention and community engagement framework to reduce their isolation and build community through engaging programs and activities
Transitional Housing Program (THP)
- one year transitional housing program with case management supports
- residents expected to work with case managers on goals, especially those related to finding and maintaining housing
- Eligibility
- Housing - Adults 18 years and older, with a history of chronic homelessness
Transitional Housing Program- all genders, 18 years and older
- homeless at intake
- high utilization of Emergency Room (ER), hospital inpatient or Withdrawal Management Service (WMS) use: 20 or more ER admissions in the past year; or, 5 or more ER visits in the past 2 months; or 8 or more WMS admissions in the past year; or 20 or more inpatient days in the past year
- active substance use issue (and/or concurrent issues)
- able to live independently with or without personal care supports obtained elsewhere
- interest/motivation to transition from the streets; goals towards permanent housing
- clear need and interest in gaining new living skills, including skills for managing a stable housing environment (i.e. the individual is expected to not be suitable for permanent housing at the present time)
- able and willing to participate in case management, client understands and agrees to program components
- able to provide proof of status in Canada
