- Employer will submit Employers' Report of Injury/Disease, Form 7, within 3 calendar days of work-related injury or occupational disease which causes worker to seek medical attention or lose time
- Worker will file a claim for benefits by signing the Employers Report of Injury/Disease, Form 6, within 6 months of the injury
