Caledon Specialist Clinic - Ext 920

• see available medical specialists in Bolton with a referral from your primary care physician or nurse practitioner

• Ontario Telemedicine Network (OTN) video conferencing is also offered on site

Transportation - Ext 218

• accessible transportation for seniors and adults with disabilities 16+ provides accessible rides to dialysis, medical appointments, day programs, weekly shopping and more
