Community based case management, including Ontario Common Assessment of Need (OCAN) to identify individual goals and create care plan

  • assistance navigating mental health system, links to appropriate services, advocacy and support
  • specialized case management, including dual diagnosis, crisis, senior, custody release and homelessness (Housing First Program)
  • service duration 6 months to 1 year
  • does not provide counselling, therapy, medication, medical procedures, legal advice or decision making

Eligibility
Individuals 16 years and older experiencing mental health, issues must be able to participate in appointments and goal setting
