Housing First
Multidisciplinary team provides intensive case management under housing first principles for individuals living with homelessness for 6 months or longer and experiencing mental health issue; contact for information on VI-SPDAT assessment and regional By-Name List
Transitional Supportive Housing
Range of housing options including a variety of support levels for individuals experiencing serious, chronic mental health illness, who wish to live in the community- high support through McKay House includes assistance in daily living skills, problem solving or coping skills, and in accessing services; capacity 15, staffed 24 hours
- medium support through Sims House supportive housing includes active client participation in household management and increased level of responsibility; capacity 6, staffed 24 hours
- minimal support housing options available for clients with ability to live independently, includes limited staff assistance with life skills, household management and crisis management provided on-site during specific hours
Complex Care Residential Program
High support transitional residential program for clients reintegrating into the community from the hospital; supports transition to long term care, independent living or permanent group living placement- capacity 12 beds in 3 residential locations

Transitional Rehabilitation Housing Program (TRHP)
High and medium support, affordable housing options for individuals with low to high risk offences, who are experiencing significant mental, emotional or behavioural challenges; assists in transition to community life,- for patients of Ontario Shores Centre's forensic inpatient units
- independent units with intermittent staff support from 2-3 hours daily
- congregate dwellings with 24/7 housing support and forensic transitional case management
